Step 250: structured diagnostic format (error codes, nodeIds, fix suggestions)
Unified diagnostic stream merging parse errors (E01xx), annotation validation (E02xx), and strategy violations (E03xx) with severity levels, AST node references, and machine-applicable fix mutations.
